Job Description:
A company is looking for a Care Continuum Partner to serve as
the main liaison between patients, families, and the clinical team
across the continuum of care.
Key Responsibilities
Coordinate and schedule complex appointments for patients,
including procedures and diagnostic testing
Facilitate communication among the clinical care team and provide
patients with clear instructions for tests and procedures
Connect patients and families with internal resources and services
to support their continuum of care
Required Qualifications
High School Diploma/GED with a commitment to advance education
2 years of experience interacting with patients in a healthcare
setting
3 years of experience in a complex health system environment or
related field
Working knowledge of electronic medical records and scheduling
software
Knowledge of medical terminology is preferred
